The financial assessment form
You will be asked to fill in a form. The information you provide in this form lets us work out how much you can afford to pay towards the care services that you receive.
If you choose not to fill in the form then you will be charged the full costs less any free personal and nursing care payments.
You or your representative should fill in this form and return it to us.
You can ask someone else to fill in this form but you or your representative need to sign the form to verify the information.
You need to send us copies of
- bank, building society or savings account statements or passbooks
- terms and conditions of any trusts
- Power of Attorney or financial guardian documents.
